I've been using Navicat Premium Essentials with our SQL 2000,2005,2008 and a few MySQL databases with pretty decent luck. Can export to CSV from the UI as well.
Got it through the App Store

MySQL Workbench
I've been using MySQL Workbench and it works ok. The only other thing I've used is phpMyAdmin which is good, but MySQL Workbench seems slicker though I've only done basic tasks with both.
