NoteTaking App with Map Capabilities Construction

I am looking for a note taking app that i can import a PDF map in and drop pins to make notes similar to iAnnotate.

But the catch is I want to be able to use predesigned forms or templates for note collection of the area.


I am using this for construction site survey type notetaking
