Formats Double Listed in "Save As..." in PS CS5

I'm having two problems with Photoshop CS5. First, when I got to save a document as a new format, each format is listed twice in the save as menu (i.e. Photoshop, Photoshop, JPEG, JPEG, PNG, PNG, so on). When I try to save as a PNG, it takes an especially long time.

Also, a file called "ScriptingListenerJS.log" keeps coming up on my Desktop. I heard it's connected to Photoshop, but I can't seem to find a way to stop it from being created.
