Photo Stream Issues

I setup iCloud on my iPhone 4 with a secondary account specifically for iPhone purchases. I use my original AppleID for iTunes store purchases.

In the Photo app, I have two Photo Stream libraries. When I shoot a photo, the photo appears in one of the libraries. The other library is always empty.

I can clear the stream on my iPhone by turning it off/on. However, when I open and look in iPhoto 9.2, the photo is never cleared. I try clearing it in iCloud.com, but it ignored.

Any suggestions on why I can't clear the stream or why two Photo Stream libraries are appearing in iPhoto?
