Work place App

I wonder if someone could help with this,
I need an app (ipad compatable) that presents me with a page which I can input the specifics of a job (like a job requirement sheet). Then when I visit a job input the requirements (either by keyboard or touch) take pictures attach them with the job requirements and then finally email the full pack.

Is there anything what would allow me to do this i have had a look on the app store but can't find anything
