My iTunes has "Children's Music" down twice. Two songs show up in one, and another song shows up in the other. The Browser window shows both identically named genres.

How do I fix this, and how did it happen?

I bet one is "Children's Music" and one is "Children's Music ".

The second one with an additional blank. Or something like that. Stuff like that drove me crazy before.

Quite possible. How did this happen? How can I fix it? How do I prevent it from happening again?

Easiest solution is when you see something like that happen, enable it so you can select both songs or multiple songs at once, right click or ctrl+click and just rename the Genre... I see it happen from time to time, no biggy. 🙂

I couldn't find which one had the space, but I did rename the genre of the song in the 2nd genre, and that genre went away.

Thanks.
