I am able to get 4k 60htz without any problems on windows 10 only. It looks wonderful!

However the mouse so slow to move around!!

trying to play starcraft is almost impossible due to the slow mouse moves. Its not until i scale to 1280x768 does it become more fluid.

Anyone know why this is?

Here is proof i got 60hz on hdmi. even when i change it to 30hz same problem very slow mouse!!

This is a 70inch 4k btw.

If the mouse is laggy, you're not truly getting 60 Hz even if windows says otherwise. For your 2012 model you are limited by the Thunderbolt 1 port which does not have the bandwidth for 60 Hz but you said you are using HDMI which there isn't any Mac with HDMI 2.0. All macs that have HDMI since 2012 have only HDMI 1.4 which has never supported 4k 60 Hz to my knowledge, you need to use a Thunderbolt 2 port with an Iris Pro 5200+, Iris 6100+, or 750m+.
 
I agree with RoboWarriorSr above, which is what I was getting at with my question.
3840x2160 @ 60 Hz is quite simply technically impossible on that machine - via HDMI or via DisplayPort.
