I wouldn't make this thread if this was my first pair of $25 EarPods. This is my 4th pair.

I've gone through 4 pairs of EarPods in about 1 1/2-2 years, and about every 3-5 months the left earpiece tends to decline in quality. What I mean is that the sound coming out of the right earpiece tends to have a much greater output than the left earpiece, which usually has sound that will only come out of the smaller speaker, as shown below:

Am I just not taking care of my headphones, or does this happen intentionally?

Apple headphones are...fragile. Though I've found that Apple Stores are quite generous in replacing them under the one year warrantee that they come with. As long as the cable isn't visibly damaged of course.

Is that so? How can I get mine replaced? Do I need original documentation?

I don't want to have to spend another $25+ on these...and the ones on Amazon are said to be knockoffs.

If you originally purchased them from an Apple Store and have a receipt, any one of the specialists there should be able to simply swap them out for you.
