iPhone is not charging well

Every time I fully charge the battery of the iphone and disconnect from the charger it shows that its actually not fully charged. So the screen (while connected to the charger) is showing the battery fully. However, if I disconnected the charger, the battery shows its half charged and therefore loses power faster.

Any ideas about this problem and suggestions how to fix it?
