iPod iPod**Warranty**help

i have an ipod video
and the hard drive is having a problem, its going slow and i cant
add songs to my ipod because it freazes each time i try
i still have warrenty and id rather fill out warrenty form online
does anyone have a link to an ipod video warrenty form or is
there no such thing?
