I've had this happen before my GPU was replaced while only a mouse and keyboard was connected.

    Wired or wireless keyboard?

"Connected" might imply wired, but I'd just like to be sure.

    Thanks.

Wired, but regardless, that issue was about the old MPs so the same argument shouldn't be applied to the new ones.

Maybe the USB controller is the culprit here, it's a Fresco Logic separate controller and maybe the drivers are not tuned.
With the 2016 version we should get the integrated Intel controllers for USB3.
Maybe this problem will go away, but still it needs to be solved for those on the 6.1

chanma98 is saying, its ok the new mac pro fails even when its new. Because things fail. Like how people die of old age or like maybe an economy fail because economies fail or like if u fail an exam because people fail exams. Apple have gained in popularity and so its ok if their new products fail for more than a year. Or like how their g5 mac pro fail and or their mac from 1980s do not work today. Its the same as the 2013 new mac pro. because things fail

Thanks. Is your mouse connected directly to the computer or is it connected to, e.g., a keyboard's USB hub?

    Cheers.

At the time, it was connected to the KB's hub.

Is there some double sarcasm going on or am I missing something?

    Why would there be sarcasm?

I wonder if this has something to do with sound. The error always starts with fifo sound assertion or something like that, and I always remember the crash with sound on the background.

No, in my case. Adobe Encoder only, (no sound), OpenGL screensaver (no sound), Finder only (no sound)...

Logic Pro X playing an entire orchestra while graphics are crashed here.
But, i must admit that I can "hear" the crash. There is always a faint click in the sound (monitoring through headphones), just when it crashes. Just that click while playing.

I also always used to get the FIFO assertion error on yosemite when my MP crashed before the repairs.

Well after the replacement, I had insane flickering in Yosemite which was fixed when I upgraded to El Capitan. Most likely a driver issue overall.

I was reading through and saw the mention of usb and sound. I have had a few instances with my nMP in which I have plugged usb drives directly into the machine and lost sound to my wireless headset only restarting the machine fixed it. There were also a couple occasions where plugging usb drive directly into the nMP completely froze the machine.

I also get these strange artifacts while playing games occasionally and using OpenGL applications. I've seen strange flickering in finder too. Here is an extreme example from Wow. I fear the GPU is probably failing.

It probably is a combination of software and hardware problems.

I was surprised when I read the ifixit tear down of the trash can Mac Pro and it had a high repairability score. Apparently replacing most of the parts are pretty easy.
I assumed it was all glued/soldiered on nonsense that apple tends to build now days. Have you found that to be true?
I still prefer the older casing for the MP.

That little flicker when logging in... it seems to have gone with 10.11.2. Hopefully a good sign. Assuming it doesn't return :)
