I know that it is okay to keep the adapter plugged in when the computer is on because it won't continue to charge the battery unless it's under 93-95%. But what about when the computer is on sleep or is turned off? How will the computer be able to monitor the battery then? Should I unplug the adapter and run it off the battery when the computer is on sleep?

It won't hurt it to leave the laptop plugged in all the time.

It knows what it's doing and won't overcharge the battery.

I believe that circuitry is in the battery rather than the computer, so if it's off, it won't overcharge. This is how you can check the battery level by pressing the little button and watching the lights.

You really aught to just leave your computer plugged in whenever you can. The batteries are smarter than you - they know how best to take care of themselves.
