Hey guys, can someone review these two pictures one taken with the 1X regular lens and the other with the 3x zoom lens and tell me why I consistently see a black dot on the upper left thanks. I obviously cleaned all the lenses a few times.

Most likely dead pixels on the image sensor. Each lens has their own sensor, which is why you only see it when using the 3x camera/lens.
 
Would back glass repair fix this or it's deep into the camera housing? I do have device insurance from my carrier and they include front glass & back glass repair

All that is necessary is to remove the display and the remove the camera assembly. Then they can blow away the debris and reassemble the phone. No front or rear glass replacement is required. Not sure if your insurance will cover that or not. What phone are we talking about?
