I have received this twice and wonder if anyone knows what it is...

Weird. All I know is Equifax is one of the "Big 3" credit reporting agencies.
Ya that's why it kinda freaks me out.
So, what are we looking at here? Is this an email? A message? What is it.
it looks like the phone is being set up though some company server, kinda the way blackberry devices are set up through enterprise software.
looks like this menu is being sent to the phone via IMEI or something, as if a worker for Equifax needs to view sensitive internal information, therefore these are the security certificates sent to the phone from company servers.
It came up as an alert like a text message or email would.
Is your phone jail broken? And if so have you downloaded anything new recently (not from the App Store?)

Not to belabor the obvious, but you might want to contact Equifax...
That's the screen that comes up when you connect to an enterprise network, that requires a certificate on the device.
I'm connected to my own password protected Wifi
Why would my phone try to do that?
Security on the enterprise network.
lol like i said, however everyone else ignored me and continued to be mystified by this simple explanation.

I saw your post but don't really understand it. My phone was bought new never had this message until a couple weeks ago. Nothing new done to it!
I think Doctoresno is on track here. I totally didn't see this post originally. You probably have an exchange server email setup that must have recently changed their certificates or something along that line.
Did you buy your phone second hand? Try restoring it...

So do I need to do something to fix it?
What does it show when you press the "Meli" button? Do you have any profiles installed on the device?
See if there's anything in there to help you remove the certs.
Also, it looks like there may be a VPN set up. Is it set to on in the menu settings?
Meli is just what I call my yahoo email
There's nothing under settings called VPN.
