Bug in iPadOS 16.x - Safari/OS not offering up passwords when logging in

Has anyone else noticed that ever since iPadOS 16 (or maybe 16.1) that the OS is hit-or-miss with regard to offering up known-login/passwords for many websites? It used to work perfectly in past OSes. This bug has persisted for multiple OS updates.

I use 16.3 in iPad Pro M1 and never experience this issue.

Adjacent to this bug is that it also won't offer fill-in answers every time for basic info (name, address, email, etc).

And this is happening across two different iPads, both on latest iPadOS. One is a 10.5 Pro and the other is a 12.9 M1 Pro.
