I'm not quite sure what this means. It has popped up on my screen when I touch two positions on the screen at the same time (I'm not sure where I touched).

But now it happened when I said: "Hey Siri".

This is on an iPhone 12 Pro with iOS 16.5

Any ideas?

I often see this appear when my iPhone is connected to my car via Bluetooth. I think it means it’s in “car mode” or “driving mode”.

No, I'm sitting in my living room and it is on the arm of the chair. I said "Hey Siri" and this popped up.
Curious, do you have another device that was used in the car during this time? Perhaps go into the settings of Focus and uncheck "share across devices". Or delete the Driving Focus (it can be reactivated again).

You may have accidentally activated the driving mode by this. If you press longer on the home screen, your iPhone lets you select a focus mode manually.

I don't have driving focus set up. And this iPhone I'm asking about is what I carry, so, it would be the device that gets plugged in to the car.

This phone is a bit glitchy when plugged in to my car. I'm wondering if it doesn't see its unplugged from the vehicle.
 
Have you checked the Bluetooth setting on the phone to see if it’s connected to the car? I know you said you plugged the phone in the car but it may be connecting Bluetooth automatically also.

Good idea! But no; there is no bluetooth connectivity to the vehicle.
 
If you are still experiencing the issue, try disabling Bluetooth temporarily.
If that helps, re-activate Bluetooth and inspect your connected Bluetooth devices.
Tap i and then Device Type to check if one (by error) is set to "Car Stereo" instead of "Speaker", etc.
