On a PC iTunes pulls the information from the built in Address Book or MS Outlook.

    Am I missing something here?

You back up your phone via Itunes, PC or MAC.

    You get new phone.

You restore via Itunes to new phone. Contacts, tunes, apps, everything goes to new phone.

    Done.

You can also sync all of that through iCloud , I do like to keep a back up on my home computer as well.

When I used Restore from Backup via iTunes for my new iPhone 5, everything came over intact, including registrations, settings, messages, photos, contacts: everything. :) It worked perfectly for my two previous iPhone upgrades. Doing it in pieces is too much work with too much margin for error, IMHO.
