I can’t for the life of me figure this out. I think in the Health app I have to connect to a health care provider to get the vaccine record. I received the vaccine from a small drug store. Which health care provider do i connect to? I assume there is a central NY State agency I have to connect to since New York supposedly supports this feature. Or am I doing this wrong? Thanks!
 
I got a QR code from CVS and just scanned that. My wife went to a random urgent care and is out of luck for now.

How did you go about getting a QR code from CVS?

There is not one on the Vaccine Record card they gave me. I also don't see it on their website when I log in with my credentials.

Did you go to the location and ask for it?

Well, the CVS started working last night for me and my wife. Got the QR code and info imported into wallet. I guess CVS was late getting to the dance.
 
On a slightly different note, has anyone been able to get the Excelsior Pass Plus through the NYS Wallet app? I can get the regular Excelsior Pass without any issues, but whenever I try to get the Pass Plus, the app is unable to find my contact info. I've reached out to tech support, but they were unable to resolve the issue. I was vaccinated at one of the NYC sites, if that matters.
 


I did this and got a pass even before apple made this a feature
