We have two unlocked iPhones. If we travel to Europe, I assume we can buy a sim when we arrive at one of the carriers and just buy prepaid service. Are there certain carriers we would have to avoid? Do you buy the sim there it before you go? Is it crazy expensive to call the states using prepaid from Europe?

You can sign up for service at any AT&T store.

T-Mobile has prepaid as well but they tend to want to charge $20 or so for the sim. They may give it for free if you sign up for service in store.

in italy I would suggest 3. remember to disable data roaming, however to avoid surprises
