Could someone who has updated answer these, please?

  1. Does OS X now remember the previously-selected state of the restore applications checkbox at reboot time so that you can choose to have your Mac boot "clean" by default?
  2. Is there any way to choose whether applications re-spawn their previously-opened windows when they start?

Some might say that I'm over-reacting, but find these behaviours too annoying to accept.
 
System Preferences > General > Restore Windows....

And no, it doesn't remember the state.

Also, you can start an app from a blank state if you hold shift while clicking on it in the dock.

Sorry, but can you explain what you mean by that? What effect does it have? I'm afraid I don't have Lion installed on anything anymore.

Thanks, I didn't know that. It really needs to be controllable on a per-app basis using the contextual options menu in the dock, though.
