Are there any real issues with the 15" screen. I've heard that it's not as sharp as it should be, in fact, grainy. How does it compare with previous 15" screens?

It is generally no better and no worse as compared to the Core Duo screens, but they are really poor compared to the 17" Macbook Pro as well as the cheap Macbook screens.

Go to an Apple store and see it for yourself. In particular avoid getting your opinion swayed by a particular poster who goes by the nick of iBorg. It is your eyes and ultimately your money. See it for yourself.
 
As said above, you should check it out for yourself. I couldn't see much difference between the 15" C2D, 17" C2D, 17" iMac, 20" iMac, 23" ACD and 30" ACD screens in my local store. However, it's possible that there are different batches of screens out there or that this is a subjective thing that different people perceive differently. Only you can decide if it bothers you or not.
