How is the first AW in these days?
Worth it ? How it’s perform at Watch OS 4?

The performance is average with watchOS 4. It still functions as it normally would, but it doesn't have the speed that Series 3 does. But it really depends on what you're using the Apple Watch for, which it still does notifications, fitness and stock applications fairly well.

When You say 'Worth it', it depends on what you want from the first generation Apple Watch. But as a safe bet, I would opt for the Series one at minimum due to the S2 processor.

I still love my original Apple Watch and use it everyday and also tracking my sleep with it (charging right after waking up).

Works fantastic. Once calibrated, workouts are very accuratly measured even without GPS on the device.

Regarding battery life: WatchOS 4.2 seems to have worsen it.
 
I wouldn't say the original watch became slower. But the Series 1/2 and 3 foremost are much faster. The Battery Life is nothing impressive and its not all day if you plan to use it for activity tracking. I would not buy it at this point in time but if you get one as a hand it down watch it will serve you well for at least one year.
If you plan on purchasing an Apple Watch I would go for a used Series 2 or 3.

Even though I still have the original watch and plan to keep it until the next iteration I can't recommend to buy one. 85$ sounds not much but the battery will start to degrade very soon for all those launch day watches and it will annoy you to charge the watch twice a day. At least you should go for a used Series 1.
 
My 38mm Series 0, bought in July 2015, is still working well. Battery is fine and performance on watchOS 4.2 is fine. Better than my old iPhone 6 on iOS 11. That said I recommend a newer model for a number of reasons. A newer model will perform better with current and future watchOS updates, provide some heart rate metrics you can't get on the Series 0, and depending on the model you can get features that might matter to you. On the other hand, if you just aren't sure if you want to pay for a newer model, $85 for a Series 0 will give you the Apple Watch experience for a very low cost. Just keep in mind it will be a little slower/less responsive than newer models, which are also low risk to try.

S0 user here, I tend to agree with most posters here, it's super slow if you want it to do the futuristic stuff (Siri, Apple Music) BUT for 2015 watch stuff like notifications and fitness tracking, the watch does it's job decently.

It's on it's way out though and I highly doubt it'll support WatchOS 5. If the time comes (June 2018) I'll save up for Series 4 to purchase come Sept 2018.

For me there will be no reason to upgrade if the new watch has no blood sugar monitor and ECG.

We'll see in the next major redesign/upgrade. It's quite due for some time now. I'll be upgrading anyway though, I think 3 years for this kind of watch is enough ROI.

Undoubtedly I believe the physical changes are will be coming with the Apple Watch Series 4 if it is in 2018. I I think the form Will remain the same, I don't believe Apple will introduce a round version, but perhaps a larger display and/or more curved.

It won’t be drastic, that’s for sure. Watch bands should be backwards compatible. Round watch is possible (see iPhone X and rounded corners).

Apple definitely will retain the band port, because they have so much invested into the band market and it's profitable. As far as a round design, highly doubt that. Even Jony Ive had stated when designing the Apple Watch, a round design did not make sense to in the future of the Apple Watch.

Rounded corners would be nice not necessarily a round watch if you get my drift.
