Background update app only four, one night without use the iphone 5s, the battery reduced 28%?
 
It can depend on many many things. What else might have been running, what kind of updates might have been coming in, what kind of signal the phone had overnight, what else it might have been doing (some sort of sync/backup/etc.).
 
Background app update is the first thing I turned off on iOS 7: very bad for battery life.
 
Background update in general drains the battery especially if the app also uses location services.
