3 iPhones, 1 Account?

Do I need to create 2 more iTunes accounts for my family that uses my iTunes account to get the free cases, or can we all use my iTunes account?

You don't have to do that. You can just download the app on each phone, and the app could tell that they are 3 different phones by the IMEI's so it shouldn't be a problem

yea i have my mums 3g and dads 3g and my 4 all on my account, works fine
