My friend's phone is acting the fool and could really use a fresh restore. he needs it to stay unlocked for T-Mobile, so what's the best route?

If the phone has the old bootrom version, create a custom 5.1 IPSW and restore it to 5.1. As far as I know, everything has been worked out on getting the jailbreak untethered and unlock working with 6.15.00 on iOS 5.1.

If it's the new bootrom, check Cydia or TinyUmbrella to see what SHSH's he has on file. Depending on what is saved and what he wants, you can create a custom IPSW and restore the phone that way.

Ultrasn0w does not yet work under 5.1 firmware
 
Sorry, I missed this thread but I know there is now an "ultrasn0w fixer" on Cydia. I had an old bootrom 3Gs come in the other day it worked perfectly.

As a personal rule, I never recommend tools from un-official hackers.
Of course each person is free to make their own choices.
