sounds like your proximity sensor is bad. have you had the phone taken apart recently? like the touchscreen or lcd been replaced? this happens alot when an inexperienced person takes apart an iPhone. this is a very difficult repair and should only be done by Apple or a good repair shop.
well I sure wouldnt pay $199 to replace it. I only charge $60 so surely you can find a shop in your area for about the same. the problem is the touchscreen has to come off the frame to replace it so there is a chance you could break the screen so you dont want to do it yourself.
