Activating a replacement 4s and restoring data

Sorry for the newbie question, but...

I got a 4s in October and a couple of days ago shattered the screen. Got my replacement today and I just want to make sure I do this right so that everything is as it was before...

I have backed up to iCloud
I have synced with iTunes

Next, I assume I activate the new phone, but what then? Do I need to swap the sim cards? If I restore from my iCloud backup, will I also need to re-sync the new phone with iTunes? If so, does it matter what order I do this in?
