My iPhone 5 will be arriving tomorrow from Apple. I've always bought from AT&T and they've always activated it in the past. Is there anything special I need to do once I open it? Like insert my SIM card from my iPhone 4? Or does Apple already take care of all that before they send it? Any help would be appreciated!

when you turn it on, you can activate from within the iphone to the line it was purchased on, at which point your old sim will cease to function.
You should just be able to connect it to iTunes and it will guide you through the process. Really, you don't even need to connect it to iTunes, since you can now set up everything on the device itself.
If you're restoring from a backup of another iPhone in iTunes, make sure you back up that phone prior. If your backup is on iCloud, then you can do that on the new phone itself. Just make sure you're on wifi so you don't use up data doing it.
