This might be a dumb question but I am going to ask anyway. Apple is sending me a replacement iPhone 5 and I was wondering what do I need to do to activate it?

Do I just go to the Verizon site and enter in my MEID or do I need to put my old SIM in the new phone or both?

You take the sim card out of the old phone, put it in the new phone, select your carrier as verizon on the new phone, and it will auto activate. You should not need to do anything at that point.
 
just make sure, 100%, that when it prompts you to input the carrier, you correctly select Verizon. if you select any other one by accident, it cannot be undone and the phone will be useless to you and they will need to send another one.
