Address Book Printing Labels

I am having a strange issue with taking addresses and printing them to labels. I imported them from Excel and they appear to show up correctly when I look at the details. however, when I go to print them they come up twice. One entry has the name, city state and zip. no street address. The second entry comes up with the name and street but not the city, state and zip. What am I doing wrong?
