My significant other has very small ears and finds even AirPods are painful to use for long periods. AirPods Pro are sadly out of the price range.

Is there a decent quality AirPods clone that might suit? Ideally with the same design approach, so a charging case and decent battery life. She'll only use them with her iPhone so there's no need for the clever switching the AirPods does.
 
There are loads of third party companies selling different size ear tips in different materials on Amazon for AirPods. My sister found a set that worked for her.

For standard AirPods? As mentioned an upgrade to AirPods Pro is off the cards because of the price.
 
That I'm not 100% on this but other AirPods need eartips. Maybe not such a big choice but a choice. A look on Amazon will answer your question. AirPod 2s in the sale are usually £179. Probably even cheaper over the coming months as the AirPod Pro 3s get closer to release.

Tips are only for the AirPod pros, the others can't take tips, and if they did, they would not make them smaller. Airpod 2s would still have the issue of being too big.

Check out the beats line, there are a few options at different price points and form factors, and you get the benefits of apple's wireless chip (fast iOS pairing, sharing across apple devices, and find my), most of them take tips, so you have some size adjustment.
