I want the 4S to connect using 2.4GHz wireless N and have the iPad 2 and MacBook use the 5.0GHz wireless N. Is the last setting the best choice for this?

That looks to be the right setting. As the AirPort Extreme is dual band, you need to set it to show the 5GHz and the 2.4GHz bands separately. Follow these steps:

  1. Open up AirPort Utility and select your AirPort Extreme Base Station (AEBS). Select 'Manual Setup' located in the lower left hand corner.
  2. Navigate to the 'Wireless' tab and select 'Wireless Network Options...'
  3. From this menu, check the box marked 5GHz Network Name and proceed to name your 5GHz band so you can identify it.
  4. Fill in the rest as per your liking.
  5. Resume normal setup procedure.
 
Yes, that's the way to go.

Or... you can leave it on automatic. After a few months with setting it the way you did I switched back to automatic and discovered that all devices (iMac, iPhone, iPad, Airport Express and the Airport Extreme itself) did a pretty good job in selecting the right WiFi-speeds/bands.
