When searching for Mac software in the future, look at VersionTraker.com and MacUpdate.com. VersionTracker.com also keeps track of software for Windows, the iPhone, and PalmOS.
In the case of forms, the best forms app for the Mac is probably pdf-Office Professional. Adobe Acrobat Pro does a similar job, albeit somewhat less elegantly. That said, you don't need a dedicated forms application to create computer-based forms. Businesses and governments now use spreadsheet-based forms--typically Excel spreadsheets. Word-based forms are also popular. As a Mac user, you have the option to purchase Pages and Numbers. They have superior layout tools that can be used to produce lovely forms.
