I found the Mystique them on Cydia, its perfect for me except for 4 icons.
The Mail, Phone, Safari, and iPod icons are missing the "box" square part. (see pic)

I just want a theme w/ transparent icons (like Mystique), but w/ all the full box icons.

I'm not sure how iPhone themes work, but if you have access to the image files (which I assume you do), it ought to be super easy for anyone with Photoshop proficiency to add the boxes to those four icons.
 
'Gruppled' on Cydia is really nice. its def. what your looking for. :)

Edit: I found out there arn't any dock icon names.

Quick question - what does the "Gruppled Lockbackgrounds" option do?
 
Well the theme comes with a couple background I THINK. So I think when you check that off in winterboard it uses the backgrounds that came with the theme.
Not positive though.

And why would you want the icon labels for this theme. The point of it is because its a clean theme that is see thru so you can use a nice wallpaper with it also.

I dont want the icon labels, I use the theme w/o labels.
I was just curious because when I turned off on the labels it didnt do it. I thought there was an issue w/ my winterboard.
 
Reveal is transparent icons with only titles. Vivid is transparent icons with little pictures, such as the ones that are shown in your pic.
