Anyone know of a simple app that lets me shutdown the iPhone to allow use of only the phone?

Specifically for when my teenager is grounded, but still needs the phone available for emergencies.

As an aside, I can't believe the isn't already a one slider on/off choice under "Restrictions" in iOS...

This is not somethhing you will easily be able to accomplish. A better plan would be to purchase a dumb phone and let her carry that when you want to restrict the device. Alternatively you can install a complex series of group policies.
 
Get Apple Configurator (free from App Store), and then use that to configure a profile on your teenager's device. You can lock down to apps etc.

Sounds like you just need to change the passcode? Your teen can still make emergency calls to you or anyone else you nominate on the emergency list.
