Apple AT&T pre-order guide

Hey all,

I just wanted to see what is needed to order for AT&T from Apple's website. It has been since the 4S since I have done this. Is it just the number I want to upgrade and the last 4 of the account holder's social? Or is there additional information needed for my AT&T account to be able to order the phone?

That is what they asked me at the Apple site when I did a "trial" update.

If the is a user passcode on the account you might want to have that handy, too! When on the AT&T website they must have this to get to the account
