Yes, but you must take the iPhone in to an Apple Store to be inspected first.

Yes, but that's only if you're talking about Applecare Plus.
Regural Applecare all you do is punch in the number that's inside the applecare box into apples website and your phones serial number and your warranty is extended another year.
