If I order my iPhone 4 from AT&T, do I have to call AT&T if it is DOA? I would much rather call Apple for customer support inquiries.

Either, but planning to call about a possible DIB* is a lot like spending your lotto winnings. :D

Dead In Box is correct term. ;)

I personally would take it to apple. its gonna be tough trying to get a replacement from AT&T
