We're considering the purchase of an Apple TV but have a few questions. My iTunes login is the same as my iCloud. However my wife's is different.

So if we purchase an Apple TV which account do we sync it with? Should we make a new account for just the Apple TV? If we make a separate account for just tv/movie purchases can we do home sharing to listen to purchased music on both of our accounts? Can you do home sharing to view photo streams?

How have others handled this - iTunes vs. iCloud accounts?

AppleTV uses HomeSharing to connect to your existing itunes libraries.
You would connect it to whatever account you feel is the most appropriate.

Photostream uses your iCloud login, but at the moment it appears you can only connect to 1 Photostream (and not multiple streams).
