Last night, while asleep, my watch that I was wearing for sleep tracking called 911. I woke up hearing the 911 operator. I told her I must have hit it by mistake and we ended the call. Fast forward to a few minutes ago, about 10 hours later, I was sitting in my chair watching TV and my watch makes the SOS Alarm and starts to call 911 again! I stopped it early. I checked my settings and “Hold Side Button to Dial” is disabled.

Is this a known issue?

All I can think of is you must do a lot of thrashing and moving while asleep and maybe you hit you watch on the night stand. My better half has socked me a couple of times while sleeping. That diamond ring I just paid for left a mark on my face!

I worked my Ultra out pretty hard in October, a total of 60 hours or so submerged in Saltwater never deeper then 100 feet. I always rinsed in fresh water, but after my first 10 hours or so of diving the action button became unpredictable. It would press itself repeatedly by itself... turning on the siren or trying to call SOS by itself. The problem was sporadic but occurred at least several times a day. I took the watch back to apple, there had been no water incursion and they did replace the watch. I have had no problem with the replacement watch, but I have not been back in the ocean. Your problem sounds like it might be something similar to mine.
 
I had it happen on my SS4 a couple of times - it was freaky and I wasn't wearing the watch and the phone was turned off. I never figured out why it happened
