I bought AppleCare+at the time of purchase in the store. I didn't get a box or anything but it is on my receipt. Was I supposed to get anything?

I'm pretty sure it sits on your account / phone serial number. So when you take it in for repair or anything it'll be on their database that you have Apple Care+

I got a receipt and an email a few hours later. I did purchase my applecare+ at an AT&T store on 10/15 after getting my preorder phone on 10/14.
 
You don't get a box; but if you ask for documentation, there is an insert they are giving out. AppleCare+ or AppleCare for that matter auto enrolls the device (Mac, iOS device) immediately when you purchase the device and AppleCare on the same receipt. That's a great benefit imo.

AppleCare/+ Does not cover lost or stolen iPhone's. That's what your cell phone insurance or home owners insurance covers.
