Application help is gone missing

Quite often when I try to get the Help for a particular application, after clicking on the Help menu at the top of the screen, I do not get any help related to the application. Instead I only get general help topics for OS X.

I have noticed this with several applications. The ones I remember are Adium and Photoshop Elements 6.

What am I doing wrong?
