Appointments log

I need an iPad/iPhone app that will log appointments, let me keep notes, and allow me to email those appointments/notes. Just a simple app to allow me to input customer names/numbers, what we met about, how long we met, etc.

I thought it would be easy to find an app like this, but I'm finding nothing, really.
