I know with Windows the spring and fall updates are optional. With macos is it the same or does it always show a red number saying to install the newest macos version?

Yes, it's optional...l and yes, by default macOS will always tell you via badging that there is an update pending...

I have used this command to ignore the Catalina update... and I suspect that you can do the same with any version of macOS by replacing Catalina with the macOS name in the terminal command.
