My gf and I have a shared plan, but its in her name.
I'm an "authorized user" for retail purchases and all, so I should be ok right?

Do I need any other information???

As far as I know from my dealings with all things AT&T, to do anything regarding the account you will need the last 4 of her social, her being the primary account holder.
