If I upgrade to the personal hotspot plan this month will I be obligated to keep it for the rest of my contract, or does it work on a month to month basis?

You can change your data plan (and other options at will on ATT - and you can choose whether to back date it to the last billing cycle beginning, today's date or when the next billing cycle starts.
 
If I install ios4.3 is the tethering featured already to go or do you have to setup the feature like on the phone or att site? I'm wondering because I have unlimited data plan and how will they know I'm tethering with the new feature since I have been tethering on mywi for awhile.

Not sure what you are exactly asking but here is how it worked for me...

I had the grandfathered unlimited data plan.
It was $30/month (we are talking AT&T of course).
I lost my unlimited and got the 4GB Tethering pro plan for $45/month.

The personal hotspot feature is in the Network settings in iOS4.3.
As soon as I switched plans on the web, the personal hotspot feature started working.

You will lose your unlimited forever if you do.

I never came close to even 1gig a month, I think I can keep it under 4.

Ah, you have a decision to make. To get tethering, you have change to a new data plan - and from what I understand once you do that you can no longer go back to an unlimited plan. But yes, you would log into your online account to setup tethering. You can't tether otherwise w/o jailbreaking the iPhone.

This is what I did today.

I'm not really a fan of jailbreaking anymore, so I said screw it, and did the 4gb plan, as I know I know I rarely even make it to the 1gb mark monthly.
