I would like to know if I only need to pay for a data plan and the $9.99 for the additional line for the iPhone.

Yes, This is basically what I pay. Though I do pay an additional $5 for 200 txt messages, but this is not required.

Keep in mind that the lowest 450 min call plan is not compatible with multiple lines. You will need at least the 700 min Family Talk plan.

Yes. $9.99 for additional line and $15.00 or $25.00 for data are mandatory. The rest are all optional.

Actually, according to AT&T, there is a family plan for 550 minutes:

(Max 3 lines)

Correct. I currently have the 550 minutes family plan with 3 lines. If I add another line, I'd have to upgrade to the 700 minute plan.
