I just noticed my iPhone 11 Pro Max has a small crack on one of the three cameras. I have insurance. Would I need to pay the $200 deductible and get it fixed before turning it in on my annual upgrade or will they accept it as is? The rest of the phone is perfect.

Short answer is yes. Long answer is how big is it and can they easily see it? If they can’t or don’t notice maybe no? Short story from me I traded in my iPad Pro 11 1st Gen at GameStop and it had a tiny scratch you could feel if you went looking for it but can’t really see it unless in bright lights. They gave me the reg cash amount. I also traded in a old Samsung tablet a year prior that and had a tiny scratch and they marked it as damage so YMMV.
 
ClosingRacer has it pretty well explained. IME "damaged" isn't a precisely defined 'thing'. If you think it's closer to good than damaged, I'd mark the trade in as 'good'. If they flag it then it was an honest mistake on your part. Either way, ultimately, they are the arbiters of what is good versus damaged. There's no way around that.

I’m trying to remember that we returned a phone of my wife’s that had some big scratches and maybe a smaller chip. Nothing was ever said about it. This really is a small defect which is kind of why I hate to spend 200 on fixing it.

IMO a scratch is normal wear for a device, an actual crack is not. You can try and see if they don't notice it but, in my experience cracked lenses, screen or back of phone will be rejected.

The more I think about it I will likely get it fixed a month or so before the next phone release.

If you trade it in-person you can always take the phone back and get it repaired if they notice it. If they don't, you'll walk out with a brand new device.😎 You might as well give it a try, the worse that can happen is they say no. If your planning on sending it in, then I'd get it fixed just to save the hassle
