I just bought CoPilot Live USA for $5. No additional monthly fees and doesn't use data; maps are preloaded.

I still haven't gotten TTS working yet (which is an in app purchase) but I have an email to support for that just a few minutes ago.

For an additional $19/year you can have live traffic updates and gas prices.
