ATT store-iPhone purchase process?

So if I go to an AT&T store to upgrade my iPhone 2G to the new 3GS, what does the process entail? Do they "setup" my new phone or hand it over to me in the box like when I bought my 2G? If they do the setup, what does this involve?

Any chance on getting the $18 dollar upgrade fee waived?

Finally, should I just order online from Apple?
